We enter Department of Corrections facilities and Community Corrections centers to assess risks and needs before the gate opens. HCF develops a Detailed Transition Plan, ensuring every participant has a direct roadmap to our community-based resources the moment they step out.

Success requires a change in philosophy and behavior. We provide individual and group mentoring equipped with the tools for long-term freedom. 


Housing & Health Stabilization

We provide upon approval housing placement and supportive services, which includes case management .


Employment & Education:

Skill-building and " GRIT" Certification workforce training.


Legal & Social Support:

Identification recovery, legal navigation, and life skills

Let's be real: Re-entry is not easy, and doing it alone is nearly impossible. At HCF, we believe you are more than your past. We created the New Entry Group-a specialized support network for formerly incarcerated men and justice-involved youth.


Why Focus on the "Group"?

Families and social networks are the backbone of a successful transition. It is overwhelming to find a job, a home, and your ID all at once.

We embrace true connections and build bridges across cultural gaps. Our groups cover everything from
