HCF Preventative Services
A New Entry to Re-Entry Program provides free and sliding scale transformational services and support to the formerly incarcerated teens and adults and teens and young adults aging out of foster care. The age requirement is 16-61. Under 18 will be referred to a partner agency.

HCF Preventative Services is a 501(c) (3) organization focused on the successful reintegration of justice-involved adults and young adults (ages 16-61) transitioning out of foster care, detention and correctional facilities.
We aim to reduce recidivism and approve public safety by addressing the educational, training, employment, healthcare, housing, and family needs of our participants.
Our Multi-Phase Approach:
Pre-Release & Post-Release Continuity:
We provide support and connection to community services both before and after release to ensure no one falls through the cracks.
Household Stabilization: We provide
housing placement and supportive services for subsidize and non subsidized placement. For non subsidize and subsidized housing you must meet all requirements. For non subsidized housing you must have income.

Life Skills & Navigation: We navigate teens and young adults through continuous life skills training, providing a " Path to Success" that is person-centered and responsive.
Restorative Justice Partnerships:
We partner with government and community groups to maximize resources and transform lives through professional accountability.
