The HCF Healthcare & Housing Initiative

At HCF Preventative Services, we believe everyone deserves a safe place to call home and the clinical support to maintain it. We serve teens, young adults, and adults (ages 16-61) transitioning from Foster Care, Correctional Facilities and Detention Centers nationwide. 

I. Clinical Community Support 

Our team consists of certified professionals dedicated to " Root Cause" wellness. We don't just talk about health; we monitor it. Our Community Health Workers (CHW) and Certified Peer Specialist-Parents (CPS-P) provide:

Point-of-Care Testing:

On-site Blood Pressure checks and A1c (Diabetes) screenings.

Health Navigation:

Helping you understand your numbers and connecting you to long -term medical care.

Parental Empowerment:

Specialized CPS-P support for mothers and fathers (ages 14-35) to ensure healthy household dynamics.

II. Specialized Housing & Disability Support

For adults (18-61) living with mental, behavioral or physical impairments, HCF provides Authorized HUD Housing Placement. We combine stable housing with our supportive clinical and non-clinical services to ensure long-term residency and health success.
